Parish Registration
You may become a member of Our Lady of Good Counsel Parish by completing our parish registration form. To request a registration form, please fill out and submit the web form below. The registration form will be mailed to the address that you provide. If you prefer, you may download a PDF file (using the link at the bottm of this page) of our registration form and print it out. Return the completed registration form to our office by mail or in person, or place it in the offertory basket at Mass.
